This could be due to a bug that our team has identified, which may cause higher-level headings to disappear when you add a lower-level heading to your content before a higher-level one.
To resolve this issue, you can click on the table of contents block and navigate to Settings as shown in the screenshot below:
Next, head to Block → Settings → EXCLUDE HEADINGS and confirm that you have not excluded any heading from appearing in the table.
