Assuming you are using the latest version of all the plugins and the themes (please update if not yet), it would seem like a conflict with one of the plugins or the theme you are using.

Thanks for getting back to me.
I installed the Health Check plugin and didn’t see any major errors.
Then I put the site into Troubleshooting mode with the default 2020 WP theme + Rank Math.
Unfortunately, I still don’t see the option to set the “Primary Category” on the post editor page.
We might need to take a closer look at the settings. Please include your WP logins in the designated Sensitive Data section.
It is completely secure and only our support staff has access to that section. If you want, you can use the below plugin to generate a temporary login URL to your website and share that with us instead:

Thank you for the reply.
It looks like the primary taxonomy had not been selected on your posts settings via Rank Math > Titles & Meta > Posts. I’ve gone ahead and selected categories as the primary taxonomy as shown below:
The “Select primary term” option should now be available any time you have more than a single category selected.
