1. All of your external images will be affected by that option when you enable it:
https://rankmath.com/kb/general-settings/#no-follow-image-file-links
2. You can inspect the image to check the nofollow attribute on your images. Here’s a screencast you can follow as well:

It really helps. I now understand.
Just one more question regarding “Nofollow External Links”, does it apply to links in footer of a website?
As when I active this button, I still see the link in footer “dofollow”.

That option only applies to the actual content of a post or page. It will not be applied to the header or footer so you would need to manually set the “nofollow” attribute.
