The icon should be added as a favicon.ico file and then you must have something like this in the <head> section of your header.php.

If your theme allows you to add a favicon, please check with them to determine if the favicon is being added appropriately according to Google’s guidelines.
Please keep in mind that Google may take some time to re-crawl your website once you’ve updated your favicon.

Please note, that as is stated on that same page, if you meet all the requirements Google doesn’t guarantee that it will use your favicon on SERPs.
