Assuming you are using the latest version of all the plugins and the themes (please update if not yet), it would seem like a conflict with one of the plugins or the theme you are using.

Hi,
i’ve followed your advice and used the Health Care Plugin.
After deactivating all plugins and using the Twenty Twenty Theme the problem is still the same. On a Test Installation (Copy of the final Installation i’ve posted here) i deinstalled the plugin and reinstalled it. Still the same issue.
What is the next step?

problem is solved. I start the setup guide again. After doing all the steps in the guide the fields are available.
But it’s strange. Seems to be a bug.
