I have the same problem as ticket #293203.
After going through that ticket, I understand the alt text not showed is not caused by rank math.
1) if theme does not support to display ALT content of the image on front end yet, Does it mean all images in site are not crawlable nor indexable by google?
2) In this case, how do I so image optimization in the aspect of seo, especially in alt text?

1. The images would still be crawlable. However, as the ALT text would not be present, Google would have less information about the image and this might affect the indexing of the images.
2. I suggest you get in touch with your theme developers and ask them to add support for the ALT texts so that they’re rendered on the front end. Once this would be fixed, the ALT texts would automatically get added, thus adding more value and information to the images.
