This isn’t about affected URLs, it is simply, when I am uploading the picture, it isn’t automatically adding the image title as alt.

Rank Math adds the ALT text to the images on runtime, i.e., when you add the images in a post and view the post on the front end of the website, then the ALT text would be displayed.
Could you please confirm if you’re checking the ALT text in the media gallery?

Thank you for that information.
As my colleague said above, Rank Math adds the ALT text to the images on runtime. They will only show up when you open the post/page on a browser.
The Add missing ALT attribute option in Rank Math does not add it in your image in the image gallery. Here’s a link for more information:
