Headings inside content boxes are not identified as optimized with focus keywords.
The image alt attribute is not identified as optimized with the focus keyword.

The Avada Builder is not yet fully compatible with Rank Math, so there may be some discrepancies in the SEO scores. Since the Alt attributes and headings are properly displayed on the front end, you may ignore the test for those upon editing your pages.
You may refer to this guide on how we address this type of issue: https://rankmath.com/kb/content-analysis-api/. This may require you to contact the developers from Avada as well.
However, please note that the scores do not affect your SEO or rankings. It is only used as an internal metric to give you a to-do list of things you can do to optimize your website.
