Could you please share the affected URL so we can check the images as well?
Meanwhile, please note that the Add missing ALT attributes and Add missing TITLE attributes options in Rank Math are not added to your images in the media gallery as the feature works only on the fly (applied on the front end when your page loads on the browser).
Also, images added by your WordPress theme or page builder to various sections like your sliders, thumbnails, icons, etc. will not be affected as they only apply to images added directly to your content.

Pls note – I have added the alternate text of the above images in the Media library also, still RankMath is not able to find out the Alt text in the images.

Hello,
We checked the source code of your site and found that truly those images have no alt text. It appears those images are not in the main content area of the page (post_content) but in the mobile menu of the ElementsKit widget. Rank Math only reads the image markups added to the post_content in the database to add the alt attributes.
You will need to contact the ElementsKit plugin support on how to add alt text to their mobile navigation menu.
