As mentioned in my 2nd post I disabled RankMath’s Sitemap feature and placed the sitemap files manually in the home directory in order to patch the issue. Ive now deleted those files so you can see the problem.

Hello,
I see you are using Cloudflare. Could you set it in Development mode (so its cache is disabled), resave the permalinks, and check the sitemap in an Incognito tab?

@Alberto Thank you for your recommendations. I followed your instructions and executed the following…
– Disabled Cloudflare completely.
– Re-save the permalinks in the WordPress settings.
– Viewed the sitemap in Incognito browser.
The problem still exists.

I have been able to replicate your issue. Could you check if you have server caching enabled. You can also check for htaccess file on your server and check if there are sitemap rewrite rules that might be causing an issue.

The website’s caching is disabled and is using the default .htaccess file from WordPress. Any other ideas?

I recommend check with your hosting support it since I have just checked the sitemap_index.xml URL and it stills seems to be being cached server-side. Let us know what they find.

I discussed this issue with my web host (WP Engine) and had them look into this sitemap issue. They have concluded that it’s not an issue with the cache.
I agree with them because the file is being modified and updated on a daily basis without issue. Also… myself and web host staff has cleared the entire cache of the website several times to rule out cacheing as an issue.
Cache is not the problem. Htaccess is not the problem. Do you guys have any other approaches to diagnose sitemap issues?

Hi Sean,
I went to WordPress Dashboard > Rank Math > Sitemap Settings.
I changed the Links Per Sitemap from 200 to 10000. That removed all the 404 post sitemaps from your Sitemap Index.
In the meantime, our dev. team is also looking into this because we cannot seem to be able to replicate this from our send.
