Rank math only generates the alt attribute for your images. This is the image attribute that is required for SEO. The title attribute on the other hand is not required and has no SEO effect(it is only available to to pointing devices:mouse)

I also wanted to that although Rank Math helps you add missing title tags to your images, using the alt test as your image title tag is not available.

So the setting called “Add missing title attributes”, does that only add titles to images in the media library or does it add titles to html?

It does not add them to the gallery. It adds the title to the images on the fly when a page loads. If you disable that option or disable Rank Math – the title/alts will be removed as well.
