Thank you for contacting Rank Math support.
Please confirm which variable you have used for the ALT attribute in our plugin’s settings. If you have used the %imagetitle%, it won’t work for the ALT field. You will have to use a different variable like %title% to add the post title as the ALT text or %filename% to add the image file name as the ALT text.
The %imagetitle% only works when you define a title in the settings of the image block, as the default title of the image is not used as the title attribute by WordPress:
