This is very odd, I cannot reproduce this behavior on my end. Is it possible that you have a third party media manager plugin installed on your site?
Could you please check for conflicts by trying to replicate this when you one have Rank Math activated as described on this page:

As per the instruction over at Confirming a Theme/Plugin Conflict With Rank Math I tried the Troubleshoot mode and even though all the plugins were disabled I am facing the same issue.

It seems like there is a conflict between Rank Math and ResizeFly plugin that you have enabled on your website.
Can you please talk to the plugin developer of that plugin and see if they can take a look?
In the meantime, we are checking this issue as well. However, we cannot replicate this on our test install and it seems like it is happening on your installation only for some reason.
Edit: I was able to replicate this. It is happening only on images that you uploaded while ResizeFly was activated on your website. Please raise a new issue with the developer here: https://github.com/alpipego/resizefly/issues

I have one more concerned at this moment when i am logged into admin, i am not able to see Rankmath under Installed Plugins, i mean it’s nowhere on the list (Inactive, Recently Active, etc) and when i try to install a new one, i get a error “Installation failed: Destination folder already exists.”
So can you check for this and help me out. Looking forward to hear back from you soon..!!

Thanks for your quick response, whenever I upload new images it is having the issue. For the above images I have regenerated thumbnails so the are showing up. Currently it is showing me as https://d.pr/i/MUoKk9.
