We have determined this is not a plugin conflict.
Is there a setting in RankMath that designates a default target image to menu items?

Could you please confirm if this issue is actually coming from Rank math? To determine this, you can disable Rank math temporarily then check if the issue still persists.
If the issue is really coming from Rank math, then We might need to take a closer look at the settings. Please edit the first post on this ticket and include your WP logins in the designated Sensitive Data section.
