The SEO analysis will detect all the images that do not have alt tags since Rank Math does not modify the data in your database. The alt tags are applied on the fly when the content is displayed on the page.
You can inspect the page source to check this.

But for the sake of logic, if the ALT parameter is ON, Rank Math WILL automatically insert the contents of all unfilled ALTs. In this case, it would be more logical not to accuse images without ALT, isn’t?
Warnings about images without ALT should only be given if the ALT parameter was turned off.
