Please note that the Add missing ALT attributes and Add missing TITLE attributes options in Rank Math are not added to your images in the media gallery as the feature works only on the fly (applied on the front end when your page loads on the browser).
Also, images added by your WordPress theme or page builder to various sections like your sliders, thumbnails, icons, etc. will not be affected as they only apply to images added directly to your content.
In that case, you may get in touch with your theme provider to add or modify the alt/title attributes.

They say it’s not part of their theme settings. They say it’s a browser setting. In all browsers I can’t find such a setting…
What I do know is that Rank Math can alter it in the ALT en TITLE settings e.g. when I choose filename. So why don’t my product images aren’t changed when I change it to %imagetitle% or %imagealt%.
