I installed RankMath with Yoast premium activated. RankMath never asked me to do a setup.
Do I need to deactivate Yoast and Yoast Premium first?

RankMath never shows up on the left after it is activated. Nor does the startup ever start when I first install it.

If the issue still persists, we might need to take a closer look at the settings. Please edit the first post on this ticket and include your WordPress & FTP logins in the designated Sensitive Data section.
Please do take a complete backup of your website before sharing the information with us.

The challenge is that the setup wizard never runs when I install RankMath.

You can start the setup wizard by going to Rank Math dashboard > Setup Wizard
Or you can try accessing this link:
https://yourdomain.com/wp-admin/admin.php?page=rank-math-wizardReplace yourdomain.com with your domain name.
If you still face any issues, please provide the WordPress logins by following the instructions shared by my colleague above.

I’ve already completed the Setup Wizard on your site. Please connect your website to your Rank Math account by heading over to your WordPress dashboard > Rank Math > Dashboard > Help.
You can also access the setup wizard on that page as well to re-setup your site.

I have not yet imported your Yoast setup to Rank Math.
You can complete that by heading over to Rank Math > Status & Tools > Import & Export.
