Thank you for contacting Rank Math support and bringing your concern to our attention.
Rank Math has options to add missing ALT and TITTLE attributes to images added directly to your content. It doesn’t do so for images stored externally. The image must also be added with the img tag and not as a background image for Rank Math to append the missing ALT and TITLE on the front end when your page loads on the browser.
