Please make sure that the Permalink structure (in Settings > Permalinks) is set to anything other than “Plain”.
I wanted to log in to your site to check this, however, you only shared the account details without the login URL (/wp-admin
& /wp-login.php
don’t seem to work on your site).
If you have verified that the permalink structure is not set to “Plain” and the issue is still happening, please share your login URL in the Sensitive Data field so we can take a look at this.

I have checked the your attachment pages and this feature is working as expected as you can see below:
I have also noted that you have selected the option to redirect attachments to the homepage and thus the new attachment links are redirected to the homepage.

Here is a printscreen that shows the url does not have the search word; yet I set the search word in the permalink as you can see. But is doesn’t get saved

Please note that the file link will not be modified, the new link will only be displayed on the SERP if you are indexing your attachments.
In regards to this error https://imgur.com/CC7a4JU it is caused by a redirection loop since you are adding a redirection to an image that has already been redirected.
You have set the global option to redirect all attachments to the products they appear in and adding another redirection on the attachment page causes this loop.
Please remove all additional redirections from the attachments to fix this.
