Assuming you are using the latest version of all the plugins and the themes (please update if not yet), it would seem like a conflict with one of the plugins or the theme you are using.

Only the site admin would know and your users won’t be affected by this troubleshooting.

I already did troubleshooting,, with default WordPress theme and only RankMath plugin ON,,, — the error persist.. the meta description and title is not updating at all.. after refresh, the meta remains same…
I read the old KB,, and I guess the issue could be with RankMath conflicting with Server.. can you confirm this?

Please check if you are using a Firewall, as there are chances that it will prevent saving the data.
If you are using a Firewall, please whitelist this route:
/wp-json/rankmath/v1/updateMeta
Whitelisting the route should fix the issue.
