Assuming you are using the latest version of all the plugins and the themes (please update if not yet), it would seem like a conflict with one of the plugins or the theme you are using.

Please let us know if you find the culprit.
Only the site admin would know and your users won’t be affected by this troubleshooting.

I did all this before. In the meantime i found the bug.
It is the “Add missing ALT and Title attributes” setting.
If i turn this off…it works.
Maybe there is a fix for this in one of the next updates?

We are checking for the issue and in the meantime, we request you to contact the theme provider/form provider as well as it is very likely that the issue might be from their side.
Your fields have this image added to them:
Rank Math is trying to add the alt/title to them which is causing the issue.
