It seems you have set your image titles to show the title of your post plus a counter. To change this please head over to Rank math > General Settings > Images and change or disable the image title attribute settings.

It is turned on for the images with the missing titles. But is applying for all the images(the images with the titles).

I want to add titles for the missing titles only, and for all the images with titles should get the original title.

If the setting is applying to any image, it is likely the image is not adding the title attribute in the end even though the title is added in the media library. To confirm this, you can disable the add title attribute functionality in Rank math then refresh and check if your images have the title added by default in the page source. If they do, let us know.

Please note that just because you have alt/title in media doesnβt mean it is showing up on the actual website. Without going into more detail, I would recommend checking the page source of your homepage and check the page source.
Look for the filename and check if there is really an alt text attached to it. If not, please check with your theme provider. If you see that particular image already has alt, please share the URL so we can check in more detail.
