Back to the issue, since the issue remains after disabling Rank Math, there must be another plugin that is causing this issue. To determine that, please follow this guide: https://rankmath.com/kb/check-plugin-conflicts/
Only the site admin would know and your users won’t be affected by this troubleshooting.
However, I checked the first URL you shared in the Sensitive Data and it redirected to the actual PNG image. Could you please confirm if this is the behavior you wish to achieve?

Yes I had to redirect permalinks to the actual images, because It’s a production site, but the problem remains. You can check with any other image on the site.
Weird thing is the problem remains even after plugins deactivation. And permalink reset doesn’t help.

Hello,
By default, the attachment URL opens the attachment page or its parent post if you enable the redirect attachments option. Redirecting to the image URL is not the default behavior of WordPress.
And even if it used to work on your site, since the issue still persists even when Rank Math is disabled then it isn’t being caused by our plugin.
