When I enable “Add missing ALT attributes” in rankmath settings and I set alt attribute format as %filename% %focuskw% %count(varname)% Will this add it to all the images in the post? ( I guess Yes)
It will only add the Alt text if the images don’t have the alt text. It will not change the alt text, which already has the alt text into them.
My concern is my posts have generally more images(average 5 and sometimes 15+ ) and If all these images have the same focus keyword in alt text , won’t that be keyword stuffing according to google ?
If you have a huge number of images that don’t contain the alt text and want to set those images to have an alt text with the Rank Math, you can tweak the setting a little bit. i.e., You can set %filename% %count(varname)%. It will not count as keyword stuffing since the file name is different from the focus keywords.
Also, You can tweak it according to your needs.

My goal is to have focus keyword in image alt text automatically generated and should not amount to keyword stuffing.
not sure if I can set this up for few images and a different setting for others?

We provide a single global option for setting the format of ALT attributes. The option for setting different formats for different images is not available.
If you have many images in a post where other contents are less and you add the focus keywords in the alt text of all the images, then that might seem to be a case for keyword stuffing.
In this case, it would be best if you set the ALT texts manually for a few images (without the focus keywords), and for the rest, let Rank Math add them.
