Could you please let us know if you are also referring to the actual Media Gallery from WordPress when you mentioned “Library”. If so, then Rank Math should be able to add the alt attributes by enabling “Image SEO” module.

Rank Math adds the ALT texts to the images that follow some rules including a rule that checks if the ALT text is not already added through the media gallery.
If the ALT text is added via the media gallery, then that is shown.
Please enable the Image SEO module as suggested by my colleague and see if the ALT text is rendered on the front end.

-below is the result of broseo.net of one page on my site, you could see the image alt text are either missing or default by theme, although I have typed ALT TEXT of all images in the Media Gallery from WordPress.

Thanks for the additional information.
While looking at your site, I noticed you have two same images, one has alt text and the other one doesn’t.
Here is the one, that has alt text:

This image doesn’t :
https://myisland.my/wp-content/uploads/2019/05/cropped-myisland-logo-cartoon-website.png
Please use the image which has the alt text. This should help with the issue.

Sorry to point that, but I do not think you are helpful.
I have about 20 images on each page on my site, and you have found two same images, one has alt text and the other one doesn’t. so you think that’s the the reason all images not showing alt text. That’s ridiculous.
How about the rest 19 images? They do not have “twin image” which don’t have alt text.
Can you dig deeper and find the real reason?

Apologies for the inconvenience caused.
I noticed that the images in the gallery have different URLs than the ones from the actual images.
To troubleshoot this, I disabled Rank Math to be doubly sure if the issue was because of our plugin.
After disabling Rank Math, the issue didn’t get fixed.
So the reason behind the issue could be something else, therefore unfortunately it does not fall under the scope of our support.
