The %imagetitle%variable works for the ALT tags but this variable is available in the PRO version.
Currently, you can enter them manually in the advanced settings of the Image block.

You can still display the alt attribute with the Free version and by default %filename% variable is set, however, the %imagetitle% variable to retrieve the image title for the alt field is reserved for the PRO version.

But i can see only 3 fields (Random Word, Image Alt, Image Title) are marked as pro in your documentation page.

Only the variables marked as PRO are not available in the Free version of the plugin.
You can also explore the available variables with the dropdown on the option to add the missing ALT tags.
