Please do note that Rank Math only adds the missing alt text to images inside the content area. We do not add it to the template sections like headers, sidebar, or footer.

Are you using any page builder on your site? If so, which one?
If you are not using any, could you please share some of the affected URLs? we might also need to take a closer look at the settings. Please edit the first post on this ticket and include your WordPress & FTP logins in the designated Sensitive Data section.

Rank Math adds the ALT text to the images on runtime, i.e., when you add the images in a post and view the post’s source on the front end of the website, then the ALT text would be displayed.
Could you please confirm if you’re checking the ALT text in the media gallery?

Yes, we don’t change the actual ALT tag in the media library to conserve server resources and only apply the necessary tag when the page is loaded to the user and bots.
