Is it possible that you are using a page builder to add images to your posts?
The RM plugin is currently only limited to HTML support and might not pick up the content in this use case. We are however in the process of introducing an API that will make it possible for page builders to be compatible with our content analysis tool.
The second issue is expected behavior since the key word can either be a single word or a phrase.

2.-A phrase may contain comma like “thing 1, thing 2 and thing 3”
