Are you using the Add Missing ALT option in Rank Math to add the ALT text?
If so, please disable lazyloading and see if that helps. When lazy loading is enabled our function that updates the tags can’t run properly on those because the function runs before any user interaction and the lazy load only runs after the first initial interaction.

Unfortunately I am using the free version of the plugin so I cannot find the the Add Missing ALT option in Rank Math. I will be trying disabling the lazyload.

I disabled the plugin that lazyloads. Nothing changed in terms of alt attributes (the screenshot is in sensitive files). Since I am a free user I cannot try the Add Missing ALT option. By the way if you can test my homepage you can see that there is no such problem with the same images. That warning can only be seen while testing the posts where the Course reviews are active.

The Add missing Alt option should also be available in the Free version of Rank Math. Please head over to your WordPress Dashboard > Rank Math > Dashboard > Modules and enable Image SEO:
Do note that some options are only available when you enable the Advanced Mode in Rank Math. If it’s not showing, you must have chosen to use Easy Mode. Please follow the steps in this link to switch to Advanced Mode:

Then, go to WordPress Dashboard > Rank Math > General Settings > Images and configure the auto-generation of your image alt attribute.
