    I also noticed that your Rank Math is disabled, to further investigate this issue. We might need to take a closer look at the settings. Please edit the first post on this ticket and include your WordPress & FTP logins in the designated Sensitive Data section.

    Thanks for providing the login details and sorry for the delay.

    I was able to login into your website’s admin area and I replicated the issue. You are using WPBakery Page Builder to display the form using shortcodes, and I noticed that you have not updated WPBakery to its latest version. Please see the screenshot available in the sensitive data section of this ticket.

    Please try updating the WPBakery Page Builder to its latest version and see if that fixes the issue. You can take a backup of your website before updating the plugin.
