To answer all your questions, by default Rank Math follows two methods for breadcrumbs hierarchy. For Pages, it uses the WordPress parent/child hierarchy. For posts it uses the category parent/child hierarchy. If you wish to customize the beyond the default breadcrumbs, you will have to use a custom filter. It seems the current theme you are using is refusing to render the breadcrumbs coming from the Elementor template. You should notice the issue is resolved if you switch to a standard default theme (example: Twenty-Twenty Two).
Can you please check with your theme provider and see if there are any settings needed to change in order to properly display the template?

I also noticed that switching theme solve the issue, so I first wrote to Elementor about this issue. Here’s their reply :
“For the breadcrumbs from Rank Math to work with the Hello Theme you will need the Rank Math’s breadcrumb widget.
I can see that in the general settings the option to add the widget is enabled, but for some reason Rank Math is not adding the widget to the Elementor editor at all.
I would suggest getting in touch with their support, since their code is out of our scope and unfortunately, we cannot tell why the widget is not loaded in Elementor, it can be that they changed the way how it is added.”
Do you know why the Breadcrumbs widget is not loading properly? Is there another issue?

The breadcrumb widget from our plugin to be used with Elementor is only available in the PRO version of the plugin.
In this case, to use our breadcrumbs you would need to implement them using one of the methods here: https://rankmath.com/kb/breadcrumbs/
In case the output from our breadcrumbs is not exactly as you would want it you can always modify the following filter to get it exactly as you want them: https://rankmath.com/kb/filters-hooks-api-developer/#change-remove-breadcrumb-items
