It seems that the problem may be related to how Rank Math dynamically applies the ALT text at runtime. When you enable the Add Missing ALT Attributes feature, Rank Math uses the filename as the ALT text for images that do not have it set in the post or page content. This means that if the images are added in a way that Rank Math does not recognize (like in headers, sidebars, or footers), it may not apply the correct ALT text from the media library.
Here are a few steps you can take to troubleshoot this issue:
- Check Image Placement: Ensure that the images are placed within the post or page content where Rank Math can apply the ALT text.
- Disable Add Missing ALT Attributes: If you want to use the ALT texts you have set in the media library, try disabling the Add Missing ALT Attributes option in the Rank Math settings.
- Recheck Media Library: Go back to your media library and ensure that the ALT texts are correctly saved for each image. You can do this by editing each image and checking the Alternative Text field.
- Clear Caches Again: After making changes, clear your server cache and browser cache once more to ensure that you are viewing the most recent version of your site.

You are adding the image using the Divi Builder and when that happens the ALT text for the image should be added to the advanced tab of the image which is currently empty:

The ALT tags should be pulled in automatically when the images have that property inside the media library so you would need to ask Divi why that is not pulling in the ALT text properly to the images.

Unfortunately, this is bad news for all DIVI users. If I understood you correctly, Divi is currently not fully compatible with RankMath, right?
Of course, we have informed ourselves in advance about the compatibility between RankMath and Divi. RankMath is even explicitly listed and recommended as compatible by the Divi developers: https://www.elegantthemes.com/integrations/rankmath/
In addition to the image module, the Divi theme offers many other modules in which images can be used. Many of these modules do not have the option of assigning an extra ALT text via the builder.
In addition, it would of course be very impractical if ALT text could no longer be maintained via the media library, but in each individual module.
In the past, it also was never necessary to maintain ALT texts in the builder. Our old SEO tool (YOAST SEO) always worked without any problems on many dozens of pages that we built in combination with the Divi theme.
We actually really love RankMath. The user interface and the range of functions are really fantastic. However, if RankMath in combination with Divi cannot output ALT texts based on the information in the media library, this is a dealbreaker for us and probably all Divi users.
Is there any chance that you will add this compatibility with an update in the near future?

After consultation with Elegant themes (Divi), the problem is already known and will be fixed in a future update. Unfortunately, they could not tell me exactly when.
The problem is not with RankMath, but with Divi.
