Thank you for contacting Rank Math, and sorry for any inconvenience caused.
Rank Math only adds missing ALT attributes, title, caption, description for images added in post or page content(inside the content editor), it will not do so for images used in headers, sidebars, footers, etc, and the images added using other plugins.
Also, the ALT text and description will be applied on the fly, Rank Math does not add these values to your database but instead applies the tag dynamically when displaying the page.
Please use your browser’s developer tools to inspect the page and check the image attributes on the source code.

0) What do you mean with “content editor”? Isn’t that where I make posts/pages?
1) Aren’t my images added with my theme Enfold added inside the content editor?
2) Aren’t my featured images added inside the content editor
3) My images are not added with other plug-ins.

WordPress default content editor is where you add content to the posts and pages.
Images added by a different theme/plugin might not be detected by Rank Math so the missing alt tag option won’t work.
A missing alt tag also gets added to the featured image when you enable the missing alt tag option.
Let us know if any image on your site has no alt tag after enabling the missing alt tag option so we will investigate further and help you out.

1) Does that mean, that all my almost 7.000 “old” images wont be handled by Rank Math?
2) And that Rank Math only handles ALT tags that are missing after enabling the function = installing Rank Marth?

Hello,
That functionality of our plugin can handle old and new images without any issue.
The only thing that it won’t handle is images that are outside the post content, such as any images you have on your header or footer or any sidebar items.
Another thing to note is that our plugin only adds the ALT tags on the fly so if you look at your images inside the media library you won’t see the ALT tag there. This is only added when the page loads.
