I’ve checked the image you’re referring to, and it seems that it is being used in the slider using ACF.
Please note that Rank Math only adds missing ALT attributes for images added directly in post or page content. It will not do so for images used in headers, sidebars, footers, sliders, thumbnails, icons, etc.

Okay, that’s what I thought. I don’t know the technical implications but would it be possible to expand the feature to images added by ACF fields?

Thank you for the follow-up, and sorry for the delay.
Unfortunately, you may need to add the ALT tag to these images manually as Rank Math Add missing ALT attributes only works with images added directly to the content.
The image you are referring to was added through a slider. Sorry to disappoint but that feature is unfortunately not available at this time.
With that being said, there are existing feature requests, so I’ve added your vote to our internal suggestions lists. If your suggestions are something that we’re able to introduce, I’ll be sure to let you know.
