Enable the Image SEO Module: Make sure that the Image SEO module is enabled in your Rank Math settings. You can do this by navigating to Rank Math SEO → General Settings → Images.

Add Missing ALT Attributes: In the same settings area, ensure that the Add missing ALT attributes option is toggled on. This will allow Rank Math to automatically add ALT text to images that do not have it.

Set the Alt Attribute Format: You mentioned setting the format to %filename%
. If you want to use the title of the image instead, you might need to adjust this setting accordingly. However, please note that Rank Math uses the filename by default for ALT attributes unless specified otherwise.

Check Image Uploads: Remember that Rank Math only adds ALT attributes to images that are added within post or page content, not those used in headers, sidebars, or footers.

I have set everything up, but when I upload an image, it still doesn’t automatically add the image’s file name as an ALT attribute. My image’s ALT attribute is blank and I have to fill it in manually, which is very frustrating!

Please note that the Add missing ALT attributes and Add missing TITLE attributes options in Rank Math are not added to your images in the media gallery as the feature works only on the fly (applied on the front end when your page loads on the browser).
Also, images added by your WordPress theme or page builder to various sections like your sliders, widgets, thumbnails, icons, etc. will not be affected as they only apply to images added directly to your content.
