We cannot replicate this on your website and the only feature that could potentially cause something like this is the automatic Video Schema generation available in the PRO version of the plugin.
Can you please share a video of the steps you took on the website for this to happen so we can try to replicate only with the Free version?

Using the press this feature/plugin and saving a YouTube video post results in 3 images, confirmed via troubleshooting wordpress feature. After testing all the plugins it points to some sort of conflict with rankmath.

I have the same issue. I went to RankMath > Dashboard > Schema and selected “Video” under Schema Type. Then, I unchecked the Autogenerate Image option, but I’m still getting hundreds of YouTube video images added to my media library whenever I update my YouTube video page, which has links to my YouTube videos (https://akistepinska.com/videos/). I’ve left the Autodetect Video option on, but should I uncheck that as well? I want the video detection, but I don’t want to generate any images.
