Since you are using the latest version of all the plugins, themes, and WordPress (please update if not yet), it would seem like a conflict with one of the plugins or the theme you are using.

Only the site admin would know and your users won’t be affected by this troubleshooting.

i have tried this and still the issue persists – it is the ability to add another in the main page when you go into the redirect itself and have source urls that is the the add another doesnt work there.
It does not create an additional row
Are you able to replicate this
