Please note that Rank Math adds missing ALT attributes only for images added in post or page content, it will not do so for images added by your WordPress theme or page builder to various sections like your sliders, thumbnails, icons, headers, sidebars, footers, etc. as they only apply to images added directly on your content, and added on the fly.
That means, after enabling the Add missing ALT attributes option, if you try to check the ALT attributes in your Media library, you’ll not be able to see this. You can check the ALT attributes from the source code of your page or by using the Inspect tool in your browser.
