Hello,
The issue here is that the page builder (Avia) you are currently using is adding the content to the database in a different way.
Rank Math only reads the image markups added to the post_content
in the database to add the alt attributes.
Right now, Avia is adding only the block shortcode and the images are nowhere to be found. See this screenshot below:

In this case, you would need to manually add the alt tag for the affected pages.
