Could you please confirm which settings are you trying to apply to your photos? Are you referring to its alt tags?
If so, kindly share the URL of the affected posts/page so we can take a closer look at it.
Also, please share the screenshot of the content analysis tests or the area where you see the error so we can further check the issue.

The problem is not related to photos! with Rank Math I adjust the focus word, title, description. and here they are after reloading the page are not saved. and only on two articles out of seventeen.

Could you confirm if you’re using a firewall? If yes, please whitelist this route and see if that fixes the issue: /wp-json/rankmath/v1/updateMeta
