I assume you are referring to this redirection option under Rank Math > General Settings > Links:
If you have already redirected it, you don’t have to worry noindexing the attachment page.
However, if you do wish to noindex it, please head over Rank Math > Titles & Meta > Attachments, enable the option “Media Robots Meta”, and set it to noindex

Is there any difference between noindexing them vs just redirecting them? Does the redirection setting noindex them?

No, redirecting them doesn’t mean you are setting them to noindex
although Google will recognize the redirection you created and acknowledges that the old URL is no longer an active content thus ranking the new URL instead.
