Is it possible that you have defined the Strip category base option on the Rank Math settings? This option can be found in General Settings > Links > Strip category base. Please note that it is not recommended to change this since the URLs have already been defined on your sitemap and would need that you resubmit the sitemap on Google for the new URLs to be crawled.

Upon checking further, it does not look like you have set up your categories to be included in your URLs at WordPress Dashboard > Settings > Permalinks.
If you want the categories to show up in your URLs, here is the permalink structure for that:
/%category%/%postname%/

Yes this will require that you resubmit your sitemap to Google so that the bots can re-index the new URLs.
