The presence of more than one canonical tag is the reason why you’re getting that error.
Assuming you are using the latest version of all the plugins and the themes (please update if not yet), it would seem like a conflict with one of the plugins or the theme you are using.

Upon troubleshooting the issue with the health-check plugin, I found that the plugin tagDiv Composer is throwing the canonical URL.
