Yes this is possible, you can head over to the media library > Select the image > Click on the “Edit more details” link and modify the advanced options from within the Rank Math metabox.

That’s strange, there is no option for modifying the advanced options within Rank Math Metabox like in single post etc. where I could make any changes or settings. There is only a checkbox showing “Exclude this image from the sitemap”.
Do you have any idea what is happening here?

We might need to take a closer look at the settings. Please edit the first post on this ticket and include your WP logins in the designated Sensitive Data section.
It is completely secure and only our support staff has access to that section. If you want, you can use the below plugin to generate a temporary login URL to your website and share that with us instead:

Please note that setting my colleague referenced takes you to the same page you have shown in your video (unless I am missing something obvious here).
You can enable the advanced option (along with other SEO options offered by Rank Math) on the attachment settings at WordPress Dashboard > Rank Math > Titles & Meta > Media.
But, please note that this setting will only affect your attachment page. It will not affect the image file itself (the one ending in jpg, png, etc.)
There is no option to mark direct image files as noindex, not using Rank Math and not with any other SEO plugin AFAIK.
You can prevent Google from crawling them using your robots.txt file.

thank you for the update.
Before having switched to RankMath I used the German SEO plugin “wpSEO”. With this plugin you have the option to choose from various robots settings and thus to mark each single image file e.g. as “noindex” or whatever you want.
This is a great feature and should be possible within RankMath as well. Are you considering implementing this feature in your plugin also? That would be great!
Preventing Google from crawling certain images by using using a robots.txt file is quite tricky, right?
What do you think about all that?

The screenshot you shared, that is for the attachment page. I already mentioned this:
I thought you were looking to noindex the .jpg, .png files.

But, even that option is not working for your site and I am not sure why. You have the Health Check plugin installed already so I assumed you have already go through the Troubleshooting mode.
