When I tried to create a test taxonomy under Hospitals or Diseases it correctly appeared under the select post type.
Furthermore, if you go to WordPress Dashboard > Rank Math > Titles & Meta > Hospitals and try to set up a primary taxonomy you’ll see that the only options are the taxonomies for that CPT.
This would indicate this is some sort of visual bug and the functionality doesn’t seem to be affected by this. What could also be happening is that the taxonomies were copied to the wrong place when you set up your relations.
