If the alt text is missing from the images added using your theme, you will need to edit the PHP files for that. Please get in touch with your theme provider and they will be able to provide you with exact instructions on that.
Also, check your view source if it is showing correctly.

Thank you for the video. I think you are misunderstanding the problem.
I am referring to image SEO as per this article https://rankmath.com/kb/image-seo/
The setting in RankMath is supposed to Add missing ALT attributes from the image title – as per the settings.
Or am I missing something?

I tested using a CPT on my end, and it worked as well.
When I tried creating a test draft post on your Listing, I uploaded an image from your post’s Add Media: Screenshot 1
Looking at the source code, I do see the alt getting created: Screenshot 2

Can you just tell me why the alt text does not show in the media library – only in the source code.

The option to add the alt text in Rank math doesn’t do this in the backend. The alt tag is added automatically when the page is opened in the front end.
What Rank math does is that when you open a page with the option to add alt attribute enabled, we look for all images that don’t have an alt attribute defined and add this according to your settings.
